FACILITY REQUEST
To schedule a meeting or event,this form must be filled out in its entirety. This includes, but is not limited to, on-going meetings, one-time events, annual events, Bible studies, outside group meetings, educational classes, rehearsals, etc... After the form is completed and submitted, it will be reviewed by the church secretary and you will be contacted within one week with a confirmation. Please do not publish or promote your event prior to receiving confirmation.
